As I stated in my last post, the folder was a mail order only item direct from Aphex's label Rephlex for £40, and it came with 12 inserts (all embossed with the Aphex logo) and No. 10 of the series, which to many people's relief is now being released individually. It was limited to 1000 copies and is not going to be re-issued (apparently). Only place you'll find it now is, of course, ebay, where it has been going for £150-200.
