The worst tram problems of which I'm aware are in central Melbourne, on the intersections where two sets of tram lines cross (Swanston St. with Flinders, Collins and Bourke Sts and Elizabeth St. with Collins and Bourke Sts, IIRC). There you have to make a right turn from the extreme left of the road!
Hook turns are being trialled in Clarendon St. South Melbourne with a view to extending their use to other suburbs in an effort to make the tram system less impeeded by traffic.
As silly as they may sound, I find hook turns work pretty well. Used to work in a building on the cnr of Elizabeth and La Trobe, looking out over the intersection. Was always fun to spot the out-of-towner who didn't know how it worked.
